Here's how it works--
1. All LNH programs start with your completed Health Intake. It is important that you are thoughtful and accurate with your responses, as they will be researched and assessed prior to your initial meeting. After completing the form, please call our office at 949.544.1166.

2. At our first meeting, we will review the evaluation of your Intake, discuss your current state of health concerns, and partner to Blueprint your personal wellness goals.

3. Coaching is an effective process to support you while you make paradigm shifts. We will coach you during weekly office visits to keep you on track to achieve maximum results in a reasonable timeframe.

The combination of our widely acclaimed, results-driven personalized Wellness Blueprint™ program and Coaching will provide you with perspective and support as you accomplish your health and well being goals:

• Change behaviors and the way you think about yourself
• Collaborative partnership; you participate in your own wellness
